“Home American Rescue Plan (Home ARP)” Case Manager and Outreach Worker

Job Description
32 Hours per week

Education/Experience:
An Associate or Bachelor’s degree in Psychology, Sociology, Human Services, Social Work or a related field is desirable. Experience working with housing and employment with unsheltered individuals preferred. Experience in working with individuals for skill building, life skill training and job seeking skills preferred.

4. An ability to analyze socioeconomic conditions and identify unmet needs of those served on both micro and macro basis.
5. An ability to support clients with job hunting or job seeking due to unemployment, underemployment, or to increase a client’s economic stability through improved employment.
6. A willingness and ability to work cooperatively with co-workers and supervisors
7. An awareness of various assistance programs and entitlements available to the unsheltered, people in poverty, and individuals with disabilities.
8. The ability to demonstrate emotional maturity with decision making in the middle of challenging or difficult events and/or situations.

1. Develop a plan of outreach into the community that will provide for consistent contact with unsheltered individuals through social service programs, the Continuum of Care (CoC) or their places of habitation.
2. Develop a network of unsheltered assistance and social service providers. Partnering with agencies to bring skill building services to the clients.
3. Identify individuals that are unsheltered that may be appropriate for Back Bay Missions Home ARP program from the COC admission referrals committee.  
4. Identify apartment, or housing opportunities to be leased for individuals coming into the programs, as well as conducting necessary assessments.
5. Develop and monitor individual service plans and housing plans.
6. Assess needs and resources of other unsheltered individuals during outreach work (who may not be served at Back Bay Mission) and assist them in accessing other forms of aid for which they may qualify.
7. Work in cooperation with other caseworkers on staff at Back Bay Mission to provide case conferencing, assistance, and resources.
8. Maintain timely and detailed statistics and client case records as required by Back Bay Mission and HUD and generate program reports as requested including necessary quarterly and annual reports.
9. Attending Continuum of Care (CoC) meetings at Open Doors Homeless Coalition.
10. Speak to mission volunteers each week.
11. Other tasks as assigned by supervisor.
